Forms, Forms, Forms, We have to do them for all the trips. Forgot them? Don't worry, the forms are here connected to this page! Here you can download our Medical Authorization and Health Form which are good for a WHOLE YEAR! and our permission form which is needed for every trip

Just download these to your computer, fill them out and return them to Blair Mathis or deposit them in the Black Forms Box in the Youth Area!

Remember that a Blue Heath Form is only good for a year, ask Blair if you are due for a new one!

	Medical Authorization Form [28kb/DOC] (BLUE FORM) Here's 1 of 2 health forms that are needed for every trip, but they are good for a year from the date signed
	Health Form [30kb/DOC] (BLUE FORM) Here's 2 of 2 a form needed for every trip that is good for a year from the date it is signed
	Warren W. Willis Camp Permission Form [718kb/PDF] This form is needed for students going on the Winter Meltdown. This is for the ropes course and after printing you need to circle both on the high and low ropes course.
